Cranial Nerve Assessment: IV. Trochlear

Examination type Neurological
Patient & Body Segment Positioning Patient needs to be in a comfortable position, either sitting or standing
Examiner Position The examiner needs to be anterior to (in front of) the patient
Performing the Test

Ask the patient to lookstraight ahead and follow your finger with his eyes as you move the finger in all planes of motion (lateral motions, up, down, in, out,  and diagonally).  The patient should not move his head.

Tissues Being Tested Cranial nerve IV. Trochlear nerve; Dorsal midbrain; eye muscles
Positive Test Inability to follow finger with eyes without moving head
Interpretation Intracranial involvement
Common errors in performing exam Not coming bringing finger forward toward the patient's nose; allowing the patient to move his head
Factors possibly resulting in misinterpretation Make sure that the athlete understands what you are doing when performing the test
