This site is intended to
serve as a repository for college and university athletic trainers to
house administrative forms, policies, procedures, position
statements/stands, consensus statements, etc. related to
all aspects of athletic health care in the intercollegiate setting.
Materials may be submitted for inclusion on this site by emailing the
documents to us or by housing
the documents on your site and providing us
with the exact web address to the document. Additionally, documents
may be mailed on CD or diskette to the address listed below. When
submitting materials please identify the purpose (medical history form,
emergency action plan, lightning policy, etc.) of the document and the
name of the institution providing the materials. Annotations and
email contact information will also be added adjacent to your link if you
desire to include them. Electronic files may be submitted in Word,
Excel, PowerPoint & pdf formats. Please feel free to submit
suggestions for additions, changes, etc. regarding this site by contacting
